INSTRUCTIONS:  If you are not able to attend an ORID meeting but would like for another member to cast your votes, complete this form and send it to the ORID Secretary by either (1) an attached e-mail file or (2) printing out and mailing it to the ORID secretary. For this proxy form to be valid, both you and the person you choose to represent you must be members in good standing of ORID.  One person can proxy for a maximum of 2 people.
